Please note that by booking an appointment and/or purchasing a package with Liz Andolong you are agreeing to the Terms & Conditions contained herein.

Liz Andolong offers private sessions in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well-being of the client.

DISCLAIMER

Liz Andolong provides wellness work in a variety of forms, but none of these healing modalities are a substitute for professional medical care or counseling. I will not diagnose or prescribe medication or medical treatment of any kind, unless I am personally qualified to do so, and will always recommend referral to a GP or other qualified medical specialist or practitioner.

The client has a duty to provide accurate and true medical and personal information, to the best of their knowledge, and to keep Liz Andolong up to date with details of any medication, symptoms, medical concerns or treatments they are having investigated or undergoing treatment for.

Liz Andolong may refuse to treat any client or part of their body with just and reasonable cause. Liz Andolong reserves the right to terminate any appointment at any time if they believe that it is not safe or in the client’s best interest to continue. Likewise, the client has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.

APPOINTMENTS

You can make an appointment with Liz Andolong in a number of ways including via our website or email. Appointments are available for a variety of durations from 30 minutes to 2 hours.

Upon making a booking, you will receive an email confirming all the important details, along with an email reminder 2 days prior to your appointment, as well as an email reminder and a text message 24 hours in advance.

If this is your first appointment with Liz Andolong, you will also receive a New Client Intake Form via the website scheduler as soon as you schedule your appointment. Please complete this because Liz Andolong will not be able to treat you until you have completed it. If it is not completed before our scheduled appointment, we will need to reschedule our session.

CANCELLATIONS & LATE ARRIVALS

Cancellation Policy

Please cancel as soon as possible if a foreseeable event is likely to prevent you from attending, for example, illness or adverse weather conditions.

Liz Andolong requires a minimum of 48 hours notice to cancel an appointment. Failure to attend an appointment with less than 48 hours notice of cancellation will incur the charge for the full appointment as she may not be able to fill your appointment slot and still need to cover costs.

However, if you need to cancel your appointment at short notice because of Covid-19 related symptoms, a long-term medical condition which varies from day to day, or because you are more than 37 weeks pregnant, then we shall discuss the cost implications on a case-by-case basis.

An invoice for any cancellation / ‘no show’ charges will be sent to you by email for payment by bank transfer within 7 days. Following a cancellation, payment must be received before any further bookings can be taken.

Late Arrivals

It is important that our agreed appointment time and duration is adhered to. Life happens and you may arrive later than expected. As you arrive, Liz Andolong will do her best to maximize the time we have left in our scheduled appointment. Liz Andolong will not be able to make up the missed time by overrunning or adding time to subsequent appointments.

Force Majeure

Every effort is made by Liz Andolong to provide the service booked by you, the customer. However some circumstances beyond our control may prevent this. Examples include (but not limited to) the following, all of which make it impossible to meet my obligations:

  • Adverse or severe weather conditions such as heavy snowfall, avalanches, floods or earthquakes

  • Road traffic accidents

  • Unforeseen traffic delays or road closures

  • Warfare and acts of terrorism (and threat thereof)

  • Industrial dispute and labor strikes action

  • Civil strife

  • Problems caused by other customers

  • Significant risk to human health such as the outbreak of serious disease

  • Natural or nuclear disaster

  • Fire, chemical or biological disaster

  • Unavoidable technical problems

  • All similar events outside of Liz Andolong’s control

If Liz Andolong’s obligations are affected by force majeure, Liz Andolong will not refund the cost of the booking, pay you compensation, or be liable.

For the purposes of these booking conditions Force Majeure means any event beyond Liz Andolong’s control, the consequence of which could not have been avoided even if all reasonable measures had been taken.

If it was not possible to provide the booked service at the time requested due to Force Majeure, Liz Andolong will try to reschedule your appointment for another time during your current stay. However, if this is not possible, a voucher of equal value to the postponed appointment will be provided for reclamation with Liz Andolong for up to 1 year.

CONFIDENTIALITY

I treat all client visits and conditions as confidential. As such, I will not discuss your visit with anybody else without your express permission.

You may choose to talk or remain silent during the session: I will follow your lead. I will periodically check in with you to see how you’re doing.

All client information and clinical records will be safeguarded by Liz Andolong and will remain confidential. Unless required to do so by law, we will not otherwise share, sell or distribute any of the information you provide to us without your consent.

Intake forms, consultations and client treatment details will not be discussed with anyone other than the client, unless the client is under the age of 18 or has a care worker or guardian, and unless expressly requested by the client.

In order to prevent unauthorized access or disclosure, we employ security measures to protect your information from access by unauthorized persons and against unlawful processing, accidental loss, destruction and damage. We have also put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online. We will retain your information for a reasonable period or as long as the law requires.

HEALTH & SAFETY

Due to the nature of realignment sessions and the need for close contact, I maintain a high standard of personal hygiene and clients are asked to ensure that they also maintain a high standard of personal hygiene. I reserve the right to wear a mask as well as ask you to wear a mask during our scheduled session. Should you have any issues with this, please be sure to bring this up before our session.

If I am unwell, suffering from a cold or virus, infection, or general ill health, I will reschedule and/or cancel my scheduled sessions due to the risks of cross contamination. This may result in directly contacting you to inform you of the issue at the earliest opportunity to make suitable alternative arrangements.

Clients are asked not to attend for a massage if they are unwell, suffering from a cold or virus, infection, or general ill health. Please contact Liz Andolong via email here or text via +1-415-323-0674 at the earliest opportunity to rearrange your appointment. If you are unwell and contact us to inform us of your illness, please note that our Cancellation Policy may still apply.

Liz Andolong will not be held responsible for the loss or damage of personal items during your time on premises that any scheduled sessions are held.

Clients under the age of 18 years must be accompanied by a parent or guardian. This parent or guardian must give permission for the agreed massage plan to be performed, and will be requested to co-sign the Intake Form. They will be invited to stay in the room for the duration of the appointment, and expected to remain present throughout the treatment.

Liz Andolong holds the right to refuse any treatment, especially if a client is physically or verbally abusive towards any practitioner, or demonstrates inappropriate, aggressive or sexual behaviour. Such behavior may result in a partial or total ban from future sessions with Liz Andolong and may also result in criminal proceedings.

COVID-19

In scheduling appointments with Liz Andolong, there will be an additional declaration form to complete stating that:

The client has not been in contact with anyone with Covid-19 in the last 14 days, to the best of their knowledge.

The client has not had any symptoms:

  • a recent onset of a new, continuous cough

  • a high temperature (typically over 100°F)

  • a loss of, or change in, normal sense of smell and / or taste

Should the client contract the virus, or be contacted by the Track & Trace service and advised to self-isolate, they must inform Liz Andolong as soon as possible.

Should Liz Andolong be exposed to COVID-19, I will ensure immediate communication with all future scheduled clients and appointments, as well as reschedule scheduled appointments once fully recovered and schedule permitting.
